Screen for Child Anxiety Related Disorders — self-report version (child/adolescent)

Who completes it: Two versions are available — choose below. The “Child/adolescent” version is self-report (the child, aged 8–18, answers about themselves). The “Parent” version is completed by the parent/caregiver about their child. For younger children, the parent can help with reading the self-report version, but should not answer in place of the child.
Instruction: “Read each statement and decide whether it is ‘Not true or rarely true’ (0), ‘Sometimes true’ (1), or ‘Often true’ (2) for you over the last 3 months.”
These questionnaires are self-observation tools and are not a substitute for clinical diagnosis by a psychiatrist or another mental health professional.
