The WHO-5 Well-Being Index of the World Health Organization is one of the shortest and most valid tools for positive mental health: 5 questions about how you felt over the last two weeks. It is freely available from the WHO. A low score may be an indication for further assessment — it is not a diagnosis.
These questionnaires are self-observation tools and are not a substitute for clinical diagnosis by a psychiatrist or another mental health professional.
